The Pet Sheet
This contains all pertinent information about your pet, you, vet and pet schedules. This document can be mailed back to us at shalaka@petsitters.co.in
Medical Certificate
This document has to be filled in by your pet’s vet after a physical check up of your pet. The Medical check up has to be done maximum 2 days prior to boarding date. Please print this document and carry it with you at the time of boarding
